I.
CLINICAL RATIONALE & CONTRAINDICATIONS
- Severe IgE-Mediated Corn Allergy: The patient has a confirmed, severe IgE-mediated allergy to corn
and all corn derivatives.
- Absolute Contraindication of Dextrose
(21CFR184.1857): Standard manufactured IV dextrose solutions (D5W,
D10W, D50W), oral glucose gels, and standard hospital juices containing
corn-derived additives (e.g., corn-fermented ascorbic acid/citric acid)
are strictly contraindicated. Administration risks severe systemic
anaphylaxis.
- Co-Morbid Condition: The patient has an established diagnosis of nocturnal reactive
non-diabetic hypoglycemia and is at risk for acute hypoglycemic crises
(Blood Sugar < 70 mg/dL).
II.
EMERGENCY HYPOGLYCEMIA MANAGEMENT PROTOCOL
In the event that
this patient presents to an Emergency Department or Inpatient Unit with acute
hypoglycemia, the following customized protocol must override standard facility
formularies:
STEP 1: CONSCIOUS
PATIENT (Capable of Safely Swallowing)
- Action: Administer patient-provided,
verified corn-free oral carbohydrates immediately.
- Approved Agents: 100% pure
unrefined cane sugar (cornstarch-free) dissolved in water, OR R.W. Knudsen
Organic "Just Concord Grape Juice" (not from concentrate, zero
corn-derived additives).
STEP 2: ALTERED
MENTAL STATUS / UNCONSCIOUS PATIENT (Unable to Safely Swallowing)
